Hip pain in the athlete has seen an explosion of growth and interest in the last 20 years. More and more athletes are being diagnosed with hip injuries resulting in pain and decreased performance. Advances in diagnostic technology and less invasive surgical techniques (hip arthroscopy) have aided the understanding of hip pathologies (including new and more recently recognized) and treatment. However, due to the nature of this rapid growth of knowledge, many team physicians are not aware of some of these newer diagnoses, the diagnostic tools and evolving examination techniques to identify these varied pathologies and/or treatments. This ICL will utilize the round table format for more individualized education in a small group format, which is ideal for this area, as different surgeons have differing degrees of baseline knowledge about hip problems in the athlete (as well as their diagnosis and management and return to play expectations), due to its relative recent growth and sporadic adoption in orthopaedic surgery training programs. These tables will be staffed by experienced team physicians who are hip arthroscopists. We will discuss early and midseason presentations of common sources of hip pain in athletes, including femoroacetabular impingement (FAI) with and without core muscle injury, hip micro-instability, dysplasia (borderline and not borderline), hip subluxation and hip dislocation. Evaluation of these varied pathologies, including examination and imaging techniques, plus non-operative and operative management will be discussed. If time allows, we will also discuss common peri-articular sources of hip pain including adductor strains and tears, iliopsoas strains and hamstring avulsions.

On-site coverage of contact team sporting events and how we manage those injuries in the first 48 hours is vital to team physicians. Who is out and who is in. What studies do we need to evaluate injuries, determine return to game play and exactly at what time that can be done safely. We will cover both upper and lower extremity issues with a focus on those injuries that are commonly seen on 'game day'. We will also discuss what changes are happening within our workspace and exactly how that may change what we do given the medical legal climate we live in. This is the type ICL that is hard to find in a textbook. Only from years of experience of on-site game coverage do you learn this specific skill set. In detail we will discuss the actual event, the first 24-48 hours post injury, and the first week of possible return to play. What can we not MISS? If surgical intervention is required, what does return to play look like and when.

This course will review key components of meniscal repair and emphasize the importance of meniscus preservation. Indications for meniscus repair versus partial meniscectomy will be discussed. Techniques for repair of complex meniscal tears, including all-inside, inside-out, and outside-in repair, will be examined in a case-based discussion along with unique advantages, disadvantages, and indications for each. Prevention with meniscal centralization will also be reviewed. Finally, meniscus allograft transplantation cases will be presented.

Upon completion of this course

• The participant will have better understanding of the role of the meniscus in knee joint health and the importance of meniscus preservation for joint longevity.

Ulnar collateral ligament (UCL) injuries of the elbow are on the rise in overhead athletes of all levels of play. Many of these injuries can be managed non-operatively with rest and a return to throwing program with or without the addition of a biologic to aid in ligament healing. However, a subset necessitates surgical intervention. Surgical techniques have continued to evolve over the past several years and now include a primary UCL repair with internal brace, modified docking technique with added graft strands, and others. Furthermore, as more primary UCL repairs and reconstructions are performed, the number of revision UCL reconstructions has risen proportionally.

The purpose of this ICL is to review the most up-to-date strategies for treatment of UCL injuries including the use of biologics, UCL repair, primary UCL reconstruction, and revision UCL reconstruction. The panel will review several complete cases and will work through each case from diagnosis to management to return to play. The participants will be taken through each case and asked how they would proceed at varying steps in management. The faculty will provide pearls based on their experience in dealing with these difficult cases. We will also discuss our UCL registry that was funded by AOSSM and provide updates.

• Understand the current risk factors for sustaining an ulnar collateral ligament injury and the most effective prevention strategies.

Much has been learned regarding posterior shoulder instability over the recent years, including its unique differences and the fact that posterior instability may be more common than anterior instability. Further knowledge regarding posterior instability's different injury pattern, symptoms, clinical presentation, bone loss patterns, and surgical treatment and outcomes have expanded exponentially. Despite this, many patients still are not appropriately diagnosed and treated. This course provides an overview of these unique issues in this cohort and how operative treatments should be approached, including surgical pearls with a case-based discussion. This panel of experts with extensive experience in treating professional athletes to weekend warriors will provide insights into the latest understanding of pathologies and treatments patients with posterior shoulder instability.

Surgical techniques for osteotomies will be presented in case-based format.

The meniscus is vital to normal knee function and the health of the articular cartilage. While meniscus tears are very common, we proportionately devote little time and resources into improving our surgical indications and technique for meniscus repair, preservation, and restoration. Recently there have been advances in recognition of tear patterns and surgical treatment of meniscus pathology. As a surgeon, it is important to understand how and when patients can benefit from these procedures. This course will provide a case-based approach to guidelines on how to recognize and treat a variety of meniscus pathology in the isolated and concomitant setting. Recognition of specific tear patterns, including full-thickness radial tears, complex tears, and root tears will be emphasized as well as indications and for meniscus transplantation. Variations of surgical techniques, such as newer all-inside techniques, gold standard repair techniques, novel centralization techniques and transplantation techniques will be covered. In addition, emerging biologic augmentation will be discussed.

This course will review knee malalignment with special focus on coronal plane and sagittal plane alignment. Indications for proximal tibia osteotomies and distal femur osteotomies will be discussed. The advantages and disadvantages of closing wedge and opening wedge osteotomies will be examined in a case-based discussion with special attention to the tibial slope and associated procedure (ACL, cartilage surgeries, meniscus transplant, etc.). There is a renewed interest in modern primary ACL repair as new arthroscopic techniques and selective patient-focused algorithms, considering tear type and tissue quality, have been adopted. Grounded in lessons learned from the historical outcomes and the most recent results of contemporary selective ACL primary repair (ACLPR), this ICL discusses the differences in the latest innovative approaches and the relevance of suture augmentation. Additionally, this ICL will review the role of the ALL and LET and the potential role of biologic augmentation. Included in this discussion will be a review of technical pitfalls and pearls. Pediatric ACL reconstruction is still a challenge for the orthopaedic surgeon. Many different surgical techniques have been developed with the goal of reducing physeal damage and re-rupture rates. In this case-based IC, a panel of European and US authors describe their indications for ACL reconstruction in the pediatric population and present a re-live surgery of their favorite surgical technique.

The purpose of this ICL is to walk the audience through successfully planning and executing a TSA and RTSA. We will discuss preoperative planning, how to determine which patients would benefit from a TSA vs. RTSA, how to perform a successful TSA, how to perform a successful RTSA. The SLAP lesion and proximal biceps dysfunction remain enigmas of sports medicine. Despite their reported prevalence, the root cause remains a mystery and recent studies indicate the posterior labrum may be the site of most of the clinically significant pathoanatomy. The purpose of this case-based instructional lecture is to provide insight and discussion into the controversies surrounding biceps and labral pathology in the athlete and active individual. This course will explore normal and pathologic appearances of the biceps anchor and labrum while developing a foundational understanding of the pathomechanics that lead to pathoanatomy. Case presentations will review both operative and nonoperative treatments in the active individual with an emphasis on the overhead athlete.

In this round table ICL, we will focus on the collision sport athlete and examine injury patterns, treatment strategies and return to play decisions in these athletes. A variety of cases from simple to complex will be presented and discussed in a collaborative fashion. We will explore topics like multi-ligamentous knee injuries, elbow dislocations, rib/sternal fractures, and recurrent shoulder instability.

Faculty will each present a case from their own practice and then turn it over to the roundtables for discussion. The group will follow up with the presenting panelist to review the outcome of the case and to identify opportunities for improvement. Cases will be presented in a rapid-fire format to maximize the number and diversity of cases that will be covered and will include athletes from all levels of play. This course will be highly interactive and engaging with ample time for discussion among faculty and participants. Audience participation will be critical, and attendees are encouraged to bring their own cases for discussion

• Describe the management of complex knee injuries, including knee dislocations/multi-ligamentous knee injuries and patella instability. Topics will include acute stabilization, definitive management, and post-operative care.
• Discuss the controversies involved in managing recurrent shoulder instability in collision sport athletes. When and why would you return an athlete to play? Arthroscopic stabilization vs open stabilization vs bone block: when why and how?
• Review the management of AC joint injuries and clavicle fractures. Should AC joint injuries be injected acutely? If so, what, when and for how long? When should surgery be considered for mid-shaft clavicle fractures? Criteria for return to play?
• Explore simple and complex elbow injuries in the collision sport athlete. Topics will include management of elbow dislocations, lateral sided injuries and UCL tears including when and why surgery would be indicated.
• Examine chest injuries in collision sport athletes like pectoralis tears, rib/sternal fractures, and SC joint injuries. Will review advantages and disadvantages of various imaging modalities, treatment options and return to play considerations.

This roundtable ICL will provide a comprehensive approach to patellofemoral joint preservation. Expert faculty will engage participants using a variety of cases that focus on the management of patellofemoral chondrosis. The moderators will present 3-5 total cases. Each case will stimulate small group discussion to formulate a problem list and surgical plan. The moderators will then present their approach in multimedia format (i.e., surgical video), provide outcome scores, and evidence-based recommendations. Strong emphasis will be placed on dynamic group discussion.

Topics covered within patellofemoral joint preservation include cartilage restoration with or without bony realignment (i.e., TTO) and soft tissue stabilization (i.e., MPFL/LRL), Indications and techniques for cartilage restoration include chondroplasty, OAT autograft, juvenile particulated allograft, MACI, osteochondral allograft, and patellofemoral arthroplasty. Our goal is to provide participants the opportunity to engage renowned patellofemoral experts in an informal setting to gain confidence in managing these challenging cases.

This ICL will cover the incorporation of Orthobiologics into the practitioners' practice, including current basic science and effective and ethical treatment options, understanding the current FDA regulations regarding clinical requirements for use of orthobiologics, and safe approaches for billing for orthobiologic practices in the office. If you are an orthopedic surgeon, it is only a matter of time before you will encounter a challenging scenario in the operating room, complex patient situation, or be faced down by a complication. The aim of this instructional course lecture is to provide education regarding some of the more common complexities, as well as more extraneous complications and scenarios that a sports medicine knee surgeon may encounter. A panel for four complex knee surgeons from academic institutions will present cases in which a complexity or a complication significantly impacted their routine of patient care. They will discuss the measures taken to rectify the problem, in addition to the values they took from their case and how it impacted their future approach to prevention and mitigation of such instances. Topics covered will include fracture and hardware failures due to implants, graft harvest, and/or tunnel malposition in ACL reconstruction, neurologic injury in the setting of meniscus repair and multiligamentous knee injuries, fracture, and failure perioperatively in the setting of osteotomies around the knee, amongst others. Case examples will include those ranging from straight forward primary ACL reconstructions to more complex osteotomies and reconstructions around the knee. Time will be given for panel discussion as well as audience interaction to fully dissect each case from multiple perspectives and experiences.

• Identify strategies in ACL reconstruction that prevent common perioperative complications such as fracture, hardware failure, impingement, and residual laxity.
• Develop problem solving strategies that mitigate risk in the setting of proximal tibial and distal femoral osteotomy for sports medicine issues around the knee.
• Understand risks factors for iatrogenic common peroneal nerve injury at the time of orthopedic knee surgery, and how to mitigate said risk factors based on a thorough understanding of lateral knee anatomy.
• Identify preoperative patient factors that may increase the risks associated with arthroscopic and open knee surgeries such as multi-ligamentous reconstruction, ACL reconstruction, and osteotomy.
